Meta-analysis of the H. pylori Eradication Rates according to the Duration of the First-line Therapy

Background/Aims: PPI-based triple therapy using amoxicillin, clarithromycin is the standard first-line regimen for eradication of Helicobacter pylori in Korea. Since the optimal duration of the therapy is debatable, we conducted this study to compare the eradication rates based on different treatment durations of triple therapy.
Methods: We analyzed two abstracts and six articles published from 1996 to 2007 in Korea, to compare the eradication rates, according to the treatment duration, i.e., 7, 10, and 14 days using Mantel- Haenszel methods.
Results: The odds ratio comparing the eradication rates based on different treatment durations was 1.91 in 7 vs. 14 days, 1.49 in 7 vs. 10 days and 1.86 in 10 vs. 14 days. The eradication rate of only 14 days treatment showed more than 90% by per protocol analysis.
Conclusions: The eradication rate of 14 days treatment was higher than that of 7 and 10 days. A longer duration of therapy resulted in a higher eradication rate. However, considering the limit of metaanalysis, we should deliberate prolongation of the treatment duration of the first line Helicobacter pylori eradication therapy prudently.
